Altru Health System is pleased to offer you member information via a Web-based version of our computer systems called MyHealth.
To request an account, submit a MyHealth Registration Form to Altru Health System, indicating you have read this Terms & Conditions Statement.
Use Of MyHealth - I understand that MyHealth should NEVER be used for urgent health care matters. The anticipated turnaround time for a response to a secure message is two business days. Therefore, if I believe I am experiencing an urgent or life threatening health condition I should phone my physician, go directly to the Emergency Department of a local hospital or call 911.
If your physician is out of the office or unavailable to respond to your secure email message, your message may be routed to another Altru Health System provider.
I understand that Altru Health System providers may send me messages via MyHealth. It is my responsibility to monitor these messages and respond in an appropriate manner. I must provide Altru Health System with a valid email address and update Altru Health System if I change my email account. Altru Health System may send me emails via my personal email address to notify me that I have new MyHealth messages. Messages sent via my personal email will not contain personal health information.
I agree that all messages that are sent through MyHealth will be regarding MY health care and conditions. I understand that asking for advice for a friend or family member is not allowed. Since these messages may become a part of my permanent medical record and will be perceived as my health information, it is important that no one else's information be reflected in my messages. With my authorization, this information may be released to insurance companies when applying for insurance. Altru Health System takes no responsibility for erroneous medical information that you provide via MyHealth that may impact your ability to be covered by insurance. Without my authorization, medical information may be released to my insurance for payment of my health care claims.
Use of MyHealth will be governed by State and Federal law.
Confidentiality - Both Federal and State laws govern the confidentiality and release of medical information. Altru Health System is dedicated to keeping my medical information confidential. Because of these laws, certain medical information may not be available in MyHealth.
Our employees are subject to strict standards on maintaining your confidentiality. By allowing you access to your medical information you must share this responsibility. Please read Altru Health System's privacy statement and Notice of Privacy Practices for more information.
Personal Identification code and Password - I understand that I will be given a unique access code by Altru Health System to allow me access to MyHealth. The first time I log into MyHealth I will be required to create a unique User ID and password. I agree to keep my user ID and password confidential. Anyone with access to my user ID and password would have access to my personal health information, ability to add to my personal health information and message my providers as if they are me. It is my responsibility to change my password if I believe it has been compromised or shared with someone else. It is recommended that you change your password every 180 days. Every time I log into MyHealth, my user ID and password will be logged in an audit trail in the computer system.
I understand that if Altru Health System discovers that I have inappropriately shared my password or that I have abused my access privileges, Altru Health System may terminate my participation in MyHealth. I understand that Altru Health System takes no responsibility for and disclaims any and all liability for damages arising from a breach of medical record confidentiality resulting from my sharing or losing my password.
Availability - I understand that MyHealth may not always be available to me due to system maintenance, back-ups or failures. Access is provided on an "as is available" basis and I am not guaranteed access at any particular time.
Minor Eligibility - An individual must be age twelve or older to request a personal account. Parents or guardians of minor children may request access to a minor child's account by following the terms outlined in the "Proxy Access" section of this Terms and Conditions statement.
Proxy Access - Individuals may request proxy access to a minorís MyHealth account by completing the MyHealth Proxy form and submitting it to Altru Health System. Such access will only be granted to parties with parental rights or legal guardianship over the MyHealth account holder and only to the extent that the party requesting proxy access can demonstrate the legal right to account holder's medical information. Under State and Federal law, there are certain types of medical information that the parent or guardian of a minor patient may not view without consent of the minor patient. Because of these requirements, some information may not be available in MyHealth.
If the proxyís legal relationship with the account holder changes, the proxy must inform Altru immediately by calling Altru Customer Support at (701) 780-6500, or sending written notice to Altru Health System, Attention MyHealth Medical Records, 1200 S. Columbia Rd., Grand Forks, ND 58201. Altru Health System reserves the right to revoke proxy access at any time for any reason.
E-Mail Privacy - Patients who are users of MyHealth should be aware that they will be notified via e-mail when there is new medical information to be viewed on MyHealth. This means that any person with access to a patientís e-mail will be able to see this notification. This could include the patientís spouse, employer or anyone else that can access a patientís e-mail account. Although no private medical information will be sent, the notification that new medical information is available by accessing MyHealth may be information that a patient would not want others to know. Thus, patients should take this into account when providing an e-mail address.
Please know that if you send us an e-mail communication, it may be shared with the Altru Health System staff that assists the physician in providing the patientís medical care. A patientís confidential medical information on MyHealth will be accessible only to appropriate clinical staff.
Altru Health System has implemented the following security features:
- Encryption: We use 128-bit Secure Socket Layer to encrypt the communication from your web browser to our secure web server. You can tell when you are secure by looking at the location (URL) field. If the URL begins with https:// (instead of http://), the document comes from a secure server.
- Dedicated web server for MyHealth: We separate the web server and electronic medical record machines. In the unlikely event that our server is compromised, no health information would be exposed since it is not saved on the web server. Patient information is stored only in the electronic medical record, behind a firewall.
- No Caching on the client side: We disable caching of data in the browser and force the web pages to expire immediately. This prevents a follow-on user from viewing pages that you had previously viewed while using MyHealth.
- Firewall: Personal health information is stored in the secure electronic medical record, protected from the Internet by a firewall.
- Session Timeout: After a period of inactivity, the logon session will time out.
- Monitoring: We constantly monitor the web server for evidence of unauthorized break-in attempts. We apply the latest security patches, as they become available.
