Activation Code Request

 

Welcome! Altru Health System is pleased to offer this online request service for your MyHealth activation code. Your activation code will be your key to your personal and secure electronic medical record. Please fill out all fields, as they are required to verify your identity.

You will receive your activation code within 4 days. If you do not receive an activation code within 4 days, please contact our Technical Support number listed above.

To request access to the MyHealth record of a patient whose medical care you help manage, please complete the MyHealth Proxy Access Form. You must complete the Proxy Access Form for any children under the age of 13 years.

Your activation code will be sent to you by email after validating your submission with your Altru Health System information. Should we have any questions regarding your submission, we will contact you via the home phone number or email address you have submitted.
